If you are unable to come for an in-person appointment, you have the option to schedule a Televisit appointment.

You will need the following:

  • Access to a phone that receives text messages and has a camera. As an alternative, our doctors can also engage in a voice call (without video) to any telephone number.
  • A short description of your eye issue.
  • Any relevant photos (i.e. visible redness, swelling, bruising to your eye) emailed to receptionist.gea@gmail.com.
  • For new patients, we will also need your address, a copy of the front and back of your insurance card, and name of the pharmacy you use emailed to receptionist.gea@gmail.com.

After your appointment has been set, you will need to download and print out an eye chart here. The directions on how to record your visual acuity is here. IF you wear corrective lenses, please wear your glasses/contacts for this. Please write down your visual acuity (i.e. Right eye: 20/20, Left eye: 20/80). This information is important to have in most cases.

If you do not have access to a printer, you can click here for an eye chart that can be read from your phone or computer screen (although the printed version is preferred). If you are still unable to obtain your visual acuity, we can still proceed with your appointment without that information.

A technician will call you up to an hour prior to your visit to set up medical record for the visit with the doctor.

Lastly, please read the INFORMED CONSENT FOR TELEMEDICINE SERVICES in its entirety. You will be asked to acknowledge that you have read, understood, and consent to the form prior to your visit.
